When a commercial truck tire is damaged, the first question is usually:
Can this tire be repaired, or do I need to replace it?
The answer depends on several important factors.
Damage That May Be Repairable
A tire may be repairable when:
The puncture is located within the tread area
The puncture is relatively small
The tire has not been driven while severely underinflated
There is no visible internal damage
The sidewall and shoulder are undamaged
The tire still has sufficient tread depth
The casing remains structurally sound
The tire should be removed from the wheel and inspected internally before a final repair decision is made.
Damage That Usually Requires Replacement
A tire will generally need to be replaced when there is:
Sidewall damage
Shoulder damage
Exposed cords
A bulge or bubble
Severe cracking or dry rot
Tread separation
Significant heat damage
Multiple improper repairs
Damage caused by driving while flat
A puncture too large or irregular to repair safely
Why Sidewall Damage Is Serious
The sidewall flexes constantly while the truck is moving. A cut, puncture, bulge, or weakened area in the sidewall can compromise the tire’s structure.
A plug is not an appropriate fix for serious sidewall damage.

The outside of a tire does not always reveal the full extent of the damage.
A tire that looks repairable from the outside may have:

Inner-liner damage
Heat deterioration
Rubber debris inside the casing

Repairing the Tire Correctly

The injury channel through the tire
The damaged inner liner
Quick roadside plugs may temporarily slow an air leak, but they should not be confused with a complete professional repair.
